The Health Information Management department at Twelve Clans Unity Hospital is responsible for the creation of, care, storage and retrieval of patient medical records. We take this responsibility very seriously to assure the utmost security and confidentiality for all our patients. We also provide statistical information for the medical and administrative staff.
Patients may request medical records and immunization records by contacting the HIM Department and filling out a Release of Information Authorization Form located here.
Please remember to fill out the authorization form completely in order to avoid any delays in receiving your medical information. Incomplete or partially completed requests cannot be processed and will be returned to the patient for completion.

Patients requesting a copy of their COVID-19 test results via secure email, please fill out the form below. Forms should be return to the HIM Department.
The Patient Advocate is available to help you during your visit at Twelve Clans Unity Hospital. If you need support, representation, or information regarding a complaint, please contact the Patient Advocate at (402) 878-3561. While you are in the clinic, you can also request that your nurse contact the Patient Advocate to come to your room to meet with you.

The Patient Benefit Coordinators at Twelve Clans Unity Hospital assist patients with third party resources. This includes confirming financial and demographic eligibility of patients, assisting with online or paper applications, making internal referrals, and providing alternative resources for patients.
Patient Benefit Coordinators can also assist with applications or renewals for Medicaid, Medicare, Marketplace Insurance, and many other programs.

In medical emergencies when a patient is away from an IHS or tribal healthcare facility or during instances when the Twelve Clans Unity Hospital cannot provide a full range of healthcare, the Purchased Referred Care Program may be available to pay for care administered by private or external healthcare providers. The PRC Program is considered the payer of last resort and requires patients to exhaust all health care resources available to them from private insurance, state health programs, and other federal programs (e.g., Medicare, Medicaid, or private insurance) before the PRC program can provide payment. There are several requirements before a patient can be authorized for the program. Please call 402-878-2231 for more information and a brochure detailing the Purchased Referred Care Program.
Social Work And Case Management
The Social Worker at Twelve Clans Unity Hospital helps patients maintain, regain or enhance their level of social functioning when their ability to function is affected by actual or potential illness, disability or injury. Some of our services include:
